SNP Overview
In general, SNPs were made for three different populations: people who are institutionalized, people who are eligible for both Medicaid and Medicare, and people who have severe or disabling chronic conditions as defined by the Centers for Medicare and Medicaid Services (CMS). SNPs come in many forms, like a Health Maintenance Organization (HMO) or Preferred Provider Organization (PPO).
Qualifying for a D-SNP
In order to qualify for a D-SNP, you need to be eligible for both Medicare and Medicaid. To be eligible for Medicare, you have to belong to one of the following: people 65 and over, younger people with End Stage Renal Disease, and younger people with disabilities. To qualify for Medicaid, you have to check your state’s criteria. Medicaid eligibility is based on income and is for low-income people. If you are eligible for both Medicare and Medicaid, you’re eligible for a D-SNP.
